The Communities of Care Program (CoC) was developed by the RI Department of Human Services as a comprehensive program to reduce non-emergent and avoidable Emergency Department (ED) use and its associated costs. The CoC program is designed for Medicaid recipients with medical, behavioral health and social needs that have demonstrated a pattern of frequent ED use. The primary goal of CoC is to decrease non-emergent and avoidable ED use through improved service coordination. All CoC members are assessed for participation in care management. RIPIN provides care coordination through the Peer Navigator. Peer Navigators are paraprofessionals with specialized training. Peer Navigators have a personal experience in special health care needs and chronic or complex illness. Peer Navigators engage with members in the home and community providing person centered, culturally sensitive support building on the values, strengths and preferences of the member.
