Care Management Program Update

Unfortunately, RIPIN is winding down our Care Management Program for Duals. This difficult decision was forced by the sudden termination of our contract with the RI Executive Office of Health and Human Services (EOHHS) under which this program operated.

We have stopped accepting new referrals into this program. If you are seeking support for someone accessing preventive services at home (e.g. homemaker services), please contact the RI Department of Human Services (DHS) at:

If you are already receiving preventive services at home and you need assistance with your prior authorization or services, please contact DHS at [email protected].  Providers with prior authorization issues can reach out to DHS at [email protected].

We are in the process of closing out our work with our current program participants. Throughout this process, RIPIN will tailor our approach to the needs of each individual participant. Wherever possible, we are making warm handoffs to other care management resources, for example through insurers or primary care providers.

Since its inception in 2018, the program has served more than 1,000 Rhode Islanders with significant medical needs and has maintained over 95% client satisfaction. In 2022, Brown University’s School of Public Health evaluated the program and found it to be associated with reduced inpatient hospital utilization, reduced long term nursing home stays, and savings to the Medicaid program of $7,000 per-member-per-year. We are extremely proud of the work of our Care Management Program and the dedicated Community Health Workers (CHWs), clinicians, and leaders who made the program successful.
