The RIPIN Care Management Program can help you stay healthy, stay active, and stay in your home by ensuring you get the care you need, when and where you need it.
Care that’s unique to your needs
We provide eligible Medicare/Medicaid enrollees a dedicated RIPIN Care Coordinator who will listen to understand your goals, work with you and your care providers, and be with you every step of the way.
Community-based support
Your RIPIN Care Coordinator can visit you in your home, provide support during appointments and meetings, and connect you with social services and programs in your community.
RIPIN Care Coordinators follow all CDC guidelines and can also provide telehealth support.
