RIPIN supports older adults, caregivers, and communities across Rhode Island by connecting people to trusted programs, services, and resources that promote healthy aging, independence, and access to care. Through our Healthy Aging initiatives, RIPIN helps individuals navigate complex systems, plan for the future, and get the support they need to age with dignity.
Our Healthy Aging Programs
RIPIN’s Healthy Aging work brings together programs that support care coordination, prevention, and individualized assistance for older adults and their caregivers.
Care Coordination & Care Transitions
Conflict-Free Case Management (CFCM)
Independent assistance for older adults and people with disabilities to explore long-term services and supports, understand options, and make informed decisions that align with their goals and preferences.
Self-Directed Support Program
Support and guidance for individuals and families using self-directed services in Rhode Island. RIPIN helps people understand their options, navigate systems, and strengthen their ability to direct services in ways that reflect their needs, priorities, and goals.
Hospital Care Transition Initiative
Short-term support for individuals transitioning from hospital to home, focused on preventing readmissions, improving follow-up care, and connecting people to community-based services.
People who are hospitalized in South County Hospital, Kent County Hospital, RI Hospital, or Miriam Hospital can request to be referred to RIPIN’s Hospital Care Transition Initiative.
Prevention & Wellness
Falls Prevention Programming
Education and support to reduce fall risk, promote safety at home and in the community, and support healthy mobility and independence.

Health Care Consumer Assistance Program
Free, confidential help for Rhode Islanders navigating health insurance and health care issues, including Medicare, Medicaid, appeals, and billing concerns.
RIPIN’s trained staff provide unbiased information and advocacy to help individuals understand their rights and options.
