Thrive Healthy Transitions

Wraparound services for individuals ages 16 to 25 living with serious mental health concerns.

The transition period from adolescence to adulthood can be challenging, especially when facing new roles, responsibilities and expectations for independence. Healthy Transitions provides a non-judgmental and encouraging environment in which to understand and work on symptom management at one’s own pace.

Thrive’s Healthy Transitions Program offers access to clinicians, a wraparound case manager, nurse, psychiatrist, substance abuse specialist, employment and training specialist who work as a team to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood. They offer wraparound services for individuals ages 16 to 25 living with serious mental health concerns.

BHDDH Adult Services Process – Students with DD

To be eligible for supports funded through BHDDH’s Division of Developmental Disabilities (DDD), individuals are required to meet the definition of developmental disability as stated in RI State Law. This agency provides an integrated, community based system of services and supports for adults with developmental disabilities (DD).

RI Secondary Transition and Employment First

In order to assist Local Educational Agencies (school districts) in meeting the Secondary Transition (to adulthood) requirements for students with disabilities, the Rhode Island Department of Education supports four Regional Transition Centers (RTCs) throughout the state.

The services provided through the RTCs include service to public, non-public, and charter schools serving youth eligible under IDEA (Individuals with Disabilities Education Act) and work in partnership with adult service agencies, higher education institutions, training programs, and parent advocacy organizations serving young adults with disabilities.

Early Childhood Special Education (ECSE): Early Intervention Transition

School districts are still required to have children transitioning from EI in service by their 3rd birthday and to report results to RIDE. The federal requirement that children transitioning from Part C (Early Intervention) to Part B have an IEP implemented by their third birthday has not been waived.

Districts should make a good faith effort to comply with the requirements during the 6-month transition period and, if eligible, to develop an IEP, which is implemented by each child’s third birthday. All districts will be required to enter EI transition information into the annual preschool performance report.

Person-Centered Planning

Person-Centered Planning is an ongoing problem-solving process used to help people with disabilities plan for their future. In person-centered planning, groups of people focus on an individual and that person’s vision of what they would like to do in the future. This “person-centered” team meets to identify opportunities for the focus person to develop personal relationships, participate in their community, increase control over their own lives, and develop the skills and abilities needed to achieve these goals. Managing Mental Health in College

College means new freedoms and new opportunities. Making the transition to college isn’t easy for anyone. Classes will be more difficult than high school and you have to plan ahead and motivate yourself to study.
