RIPIN receives federal grant in partnership with Progreso Latino and Prospect Health Services for chronic disease self-management education for older adults and adults with disabilities

WARWICK, R.I. — RIPIN is proud to announce that it has been awarded a 2022 Empowering Communities to Deliver and Sustain Evidence-Based Chronic Disease Self-Management Education (CDSME) grant by the Administration for Community Living (ACL).

The grant will support a multi-partner approach to program delivery in the cities of Central Falls, Pawtucket, Providence, and Woonsocket. RIPIN will partner with Progreso Latino and Prospect Health Services RI to expand, deliver, and sustain evidence-based CDSME and support programs for older adults and adults with disabilities, particularly those in underserved populations or geographic areas in Rhode Island.

“Almost 27% of Rhode Islanders live with multiple chronic conditions such as diabetes, hypertension, and chronic pain.” said Nancy Silva, Director of Health Initiatives for RIPIN. “We know these evidence-based programs help people live healthier lives and remain independent as they age. We are proud to have the work we have been doing recognized by ACL and excited about this opportunity to expand and reach even more Rhode Islanders.”

Since 2012, RIPIN has connected over 5,200 adults to evidence-based chronic disease self-management programs to help them take control of their health. RIPIN was one of just nine grantees nationwide. The grant will support RIPIN’s CDSME programs through April 2025.
